A new provincial policy on maximum on-call burden in the form of a Ministerial Letter
An Alternate Payment Plan (APP) is a tripartite agreement that may be negotiated on behalf of a discipline between Health and Community Services, Newfoundland and Labrador Health Services (NLHS), and the NLMA that provides remuneration in a format other than salary or fee-for-service (FFS) in return for the provision of medical services.

The Memorandum of Agreement (MOA) signed on May 3, 2022, included a commitment from the NLMA and the provincial government to establish Blended Capitation as a new payment schedule of the MOA. The final agreement was arrived at with the assistance of a mediator and officially released on April 3, 2023. As per Article 17.10 of the MOA, the Treasury Board Secretariat, Market Adjustment Policy is applicable to salaried physicians and may be used by NLHS where it determines that it is unable to recruit salaried physicians in specific positions at a particular geographic site. Subject to the approval of the Treasury Board Committee of Cabinet, NLHS may approve and provide benefits to salaried physicians beyond those benefits outlined in the MOA.

This Payment Schedule identifies the amounts prescribed as payable and rules and conditions of payment under the Physicians and Fee Regulations (Schedule A), governed by the Medical Care Insurance Act for insured services rendered by licensed physicians.
On-Call Payment Program Information Manual. On-call rates are based on a four-tiered rate structure. The rates are based on average intensity and volume of on-call work by discipline. The four-tiered rate structure treats all physicians in the same discipline, wherever they are located in the province, on the same tier.

The Government of Newfoundland and Labrador, in consultation with the Newfoundland and Labrador Medical Association, is offering a one-time Recognition and Retention Bonus of up to $25,000 to eligible family physicians in exchange for a one-year return-inservice agreement (from the date of signing). This one-time bonus will be administered by Newfoundland and Labrador Health Services, the newly established Provincial Health Authority.
As outlined in Schedule “Q” of the 2017-2022 MOA, the Rural Community Comprehensive Care Bonus (RCCCB) came into effect on September 30, 2022. Applications will be accepted on or after September 30, 2023 (covering the bonus period September 30, 2022 – September 29, 2023) which is the first full year of the new RCCCB program. The RCCCB replaces the FFS Family Medicine Rural Retention Bonus (RRB). The RRB remains in place for FFS specialists.

The NLMA will send a reminder on September 1, 2023, to complete your application, which must be submitted by December 31, 2023.

For the provision of medical services to injured workers.
The Continuing Professional Development (CPD) fund outlined in the 2023-2027 MOA (Article 19.04) is a newly negotiated benefit and will provide physicians up to $4,500 per year. All practising physicians in the province are eligible to receive this benefit based on the following criteria.
- Practising physicians who have a Newfoundland and Labrador practising license and have a permanent address in the province will automatically receive the $4,500 early in the new fiscal year for the preceding year. No application is required.
- Physicians with a permanent address outside NL, and provide a minimum of 75 days of service in NL, must apply through the designated application process through the link below to receive the benefit:
Timing and proration for this year:
This is a newly negotiated benefit that is effective as of September 2, 2025, the date the MOA was signed. Therefore, the benefit will be prorated for the current year, covering the period from September 2, 2025, to March 31, 2026 (end of GNL’s fiscal year).
- The prorated amount will be $2,601.40.
- For physicians with a permanent address outside of NL, a minimum of 43 days of service in NL for the period of September 2, 2025, to March 31, 2026, are required to be eligible for the benefit.
For physicians automatically eligible for this benefit (1 above), this payment will be issued no later than May 15, 2026 (no application required).
For physicians that must apply for this benefit (2 above), the deadline is June 30, 2026, and this payment will be issued once your application is received and processed. Any physician who did not have a permanent NL address prior to September 2, 2025, but has since moved to NL and obtained a permanent address, should still complete the application for CPD this year, as if you did not have a permanent NL address, to ensure you are not missed.
Looking ahead:
Beginning in April 2026, the CPD payment will be available as an annual benefit of $4,500, subject to the eligibility criteria outlined above.
