New Referral Form: Eastern Health Geriatric Psychiatry and Geriatric Medicine Outpatient Services

Effective immediately, the Geriatric Psychiatry and Geriatric Medicine Outpatient Services at Eastern Health have replaced their two separate referral forms with one combined form. Eastern Health is optimistic that this new form will be more user-friendly and provide an enhanced single entry for geriatric services. The new combined form is available on the intranet and is called Outpatient Geriatric Services Referral-Geriatric Medicine and Geriatric Psychiatry (Ch-2040).
