Medication Therapy Services Clinic
The Medication Therapy Services (MTS) Clinic is a pharmacist clinic run by Memorial’s School of Pharmacy that is available to physicians who wish to refer complex patients in need of comprehensive medication assessments.
The clinic also develops recommendations for physicians and plans to optimize and simplify medication regimens for patients. The clinic is beneficial for patients with multiple chronic conditions or polypharmacy; those who need help tapering or changing medications; patients who have been recently hospitalized or have had significant medication changes; and, those who have experienced new or persistent falls or cognitive changes.
