Closure of Psychiatry Referrals (Effective Immediately)

Dear Referring Provider,
The e-fax number that has been used to send referrals to me is changing. Based on past experiences with changes in my contact information, there is the chance of referrals not being received (e.g., sent to the wrong number or one that I no longer have access to). I also recognize that the change in my contact information is occurring in the midst of the switch to CorCare.
Therefore, effective immediately, I will no longer be accepting referrals. This decision is being made in an effort to minimize any gaps in patient care.
An updated referral form, which will include the contact information and referral criteria of my practice, will be circulated when I am once again accepting referrals.
