Cardiovascular Disease Prevention Workshop in PEI

Drs. Sheldon Tobe, Rahul Jain and Thor Christensen will be facilitating the second C-CHANGE PEARLS Clinical Practice Guidelines Workshop on Friday September 13, 2019 at Glasgow Hills Resort, New Glasgow, PEI.

Session Description
It is anticipated that after the session participants will be able to take away ‘practice pearls’ and implement the C-CHANGE cardiovascular harmonized guidelines in their practice to optimize screening, diagnosis and management of patients with multiple morbidities. Participants will engage in interactive small group discussions using case studies led by facilitators. Q & A sessions will follow each case study including local experts: Physicians, Nurses and Pharmacists.

 

Objectives

Upon completion of this case study, participants should be able to:

Module:  CV Risk Reduction in Diabetes (SGLT2 inhibitors/GLP-1 Receptor Agonists in Patients with DM and CVD)

  • Describe the patient who should be treated with an SGLT2i or GLP-1 agonist.

  • Explain the rationale for this treatment and the potential benefits.

  • Describe the steps for initiating and monitoring therapy for patients with diabetes.
     

Module: Hypertension and Stroke Prevention and Management in Patients
Upon completion of this case study, participants should be able to:​

  • Plan to assess and screen patient’s blood pressure at all appropriate visits.

  • Discuss modifiable cardiovascular risk factors with patients.  

  • Discuss blood pressure targets and need for anticoagulation/anti-platelet therapy in people who have just suffered a stroke.  

Module: How C-CHANGE can help clinicians manage the multi co-morbidity patient

  • Identify recommendations for multiple morbidities from the C-CHANGE guidelines.

  • Implement recommendations for multiple morbidities in a single patient.

  • Use the C-CHANGE recommendations to help keep the elderly living at home longer and healthier.
